Outcome Star – A Holistic Approach to Measuring Progress

At Venture People, we are committed to empowering individuals to move forward in their recovery journey by building on their strengths and achieving greater independence. The Outcome Star is a vital tool used across our services to support this process.

Developed  by Triangle Consulting Social Enterprise Ltd, it is a validated outcome measurement tool that helps individuals:

  • Identify their goals and reflect on their current situation.
  • Measure progress in key areas of their lives, such as health, housing, independence, relationships, and meaningful activities.
  • Celebrate achievements while identifying their next steps on the Journey of Change.

The Outcome Star is embedded into our person-centred, holistic, and outcomes-focused approach, ensuring that each individual remains at the centre of their support. By working collaboratively with the people we support, we use the Outcome Star to:

  • Tailor support plans to individual aspirations and needs.
  • Deliver one-to-one support around well-being, health, daily living skills, housing, and meaningful activities.
  • Track distance travelled, capturing progress over time and evidencing outcomes.
